New Patients
Welcome to our practice. I hope to establish a long relationship with you, your friends, and family. Here are some helpful tips on what information you need to bring and what to expect on your first visit.
- If you have dental insurance, please bring your insurance card or any other pertinent insurance information. My staff will assist you with any of your insurance questions. We accept all PPO and private insurance plans but unfortunately do not accept HMO insurance.
- You will need to fill out a complete New Patient Information form and medical /dental history. To save time, you may print these forms from our website and bring them with you on your first appointment.
- Your first visit is an important one and in most cases includes your visit to our hygienist and a complete dental examination including digital x-rays, intra-oral exam, head and neck exam. The dental hygienist will educate you on the health of your gums. Our treatment coordinator will go over the results of the examination and help you to schedule treatment. She will discuss finances with you and develop a payment method that is suitable to your needs. We have a variety of payment options including cash, credit, and other financing options.
